Introduction
CodeDoc maintains a record of all user input during the code. Each entry is time stamped. All editing and changes to the record are saved and time stamped so that the exact sequence of data entry is preserved. The record is presented to the user in a time line format, called the "master log". The "master log" shows each data entry time and a very brief summary of the most important data for each entry. The full data can be called up from the master log with a single pen press. It may be viewed to be edited to correct mistakes. Times are presented in both clock time and elapsed time (ET) so the user may quickly determine intervals and duration of events. Our docutimer allows the documenter to concentrate on documentation not on the clocking time. CodeDoc may be used in different ways. One way it may be used is to alert medical personnel of the need to document assessments of the effects of medications that have been administered. Because of the time stamped record of data entry, the program knows when the medication was administered, when the previous assessments were done, and when the next assessment is due to be entered. The program suggests time intervals for alerts based on the time about which the next dose is usually given, or the user may override these suggested alert intervals with their own values. The alerts continue at a fixed interval until the user cancels them. The time stamped record is processed to provide concise summary of the code blue, including the amount of time the patient spent in various cardiac rhythms, the amount of time spent in CPR, total doses of each medication administered and documented, and approximate total fluid input/output balance and a list of each unique medication administered with time of documentation.
